The personal data are collected, processed, filed, maintained, stored and submitted to the public authorities and are used in compliance with the law for the exercise of my profession. The contracting party, upon completion and submission of any form, agrees and consents on the above mentioned personal data processing and hereby states that all submitted data for the preparation and completion of any contract, is performed according to his mandate and with data submitted and provided by himself.
